Application Forms
IELTS Application Forms can be picked up in person from the IELTS Test Centre or you can download an application, along with a credit card authorization form:
Requirements:
Each application MUST include:
- 2 passport-sized pictures, both signed on the back. Please download Guidlines for IELTS Candidate Photographs
- All four pages of the application form
- A photocopy or scanned copy of your passport or Permanent Resident Card (PRC) showing your name, passport number, picture and expiry date
- A valid passport or PRC number on the application form
- Payment for the test fee
- Addresses of institutions where you would like additional test results mailed (if applicable)
On the day of the test, candidates are required to show their PRC or passport (the same as listed on the application form) at check-in and throughout the day. Candidates who do not bring their passport /PRC will not be permitted to write the test. If you have sent your passport to an institution for renewal or other reasons, it is your responsibility to ensure you have your passport with you on the day of the test.
Submitting your Application
Submit the completed application form and payment by mail or in person to the IELTS Test Centre. Do not fax or e-mail your application. Only original applications will be processed. Incomplete applications will NOT be processed.
Application Deadlines
The original IELTS Application Form and the test fee must be received by the Test Centre at least seven (7) working days prior to the test date. The Test Centre reserves the right to close a particular test date early if all spaces are full. Applicants who do not receive their earliest test date requested will be forward to their second choice test date.
Confirmation Required
Upon receipt of the completed IELTS Application Form and test fee, the Test Centre will email a letter of confirmation stating the testing details including test location, room, etc., usually within three days of registration. Telephone confirmation may also be made. Only those candidates who have received written or verbal confirmation will be allowed to take the test.
