For students who plan on staying in Residence from May 1, 2012 to August 15, 2012.

Partial Duration Form

For students who are staying in Residence for a portion of the semester (i.e. May 1, 2012 to July 15, 2012).

Winter Term Extension Form

The 2011-12 Residence Agreement expires as of 12 pm on Saturday, April 21, 2012. Residents who would like to remain in their allocated suite up to an additional 5 business days or less (no later than 12 pm on Friday, April 27, 2012) must submit this form.  All requests are subject to approval. Approved residents are required to pay the Winter Extension Fees and sign the Spring/Summer Amendment Agreement should they wish to remain in their allocated suite.

Residence Cancellation Form

This form is for students who wish to cancel their confirmed ($500 Security Deposit has been paid) space at MacEwan Residence.     

Returning Resident Storage Agreement

This form is for returning residents who would like to store their personal items in MacEwan Residence over the summer.

Request to Withdraw Form

Residents are encouraged to refer to the "Moving Out" section of our website prior to submitting a Request to Withdraw Form.

Work Order Request Form

The Work Order Request Form is for student residents with a mechanical problem (e.g. toilet not working, dripping faucet, etc.) in their suite. Causal guests simply need to contact the Front Desk at extension 8000 to report these issues.

Room Change Application 

The Room Change Application Form is for current residents who wish to conduct a room change.  All Room Change Application are reviewed by Residence Life Staff and approvals are subject to availability and the Steps to Complete Before Submitting a Room Change Request (outlined on page 33 of the 2011-12 Residence Handbook).

Fall Term 2011 - Room Change Applications open on Monday, October 3, 2011 and close on Friday, December 16, 2011. 

Winter Term 2012 - Room Change Applications open on Wednesday, February 1, 2012 and close on Thursday, March 1, 2012. 

F.O.I.P Consent Form

Submitting a F.O.I.P. Consent Form allows a representative of a resident to communicate with MacEwan Residence on the resident's behalf.
