Report an Incident

An incident is any event that causes, or could cause, harm or damage. This includes injuries, illnesses, equipment failures, chemical spills, fires, floods, explosions, near misses or situations where first aid is provided.

Everyone at MacEwan University—students, staff, faculty, contractors and visitors—must report incidents that occur on campus or during university-sanctioned activities, such as labs, practicums, field trips or study abroad. Reports must be submitted within 48 hours or as soon as possible if you’re unable to report within that timeframe.

Reporting helps prevent future incidents, ensures MacEwan meets safety obligations and automatically initiates Workers’ Compensation Board (WCB) reporting for students and employees when required.

If your situation isn’t an incident but involves a safety concern or a conduct or policy issue, you may need to submit a concern or complaint instead.

Select the form that applies to your role. Current MacEwan students, School of Continuing Education (SCE) students, employees, and visitors each have different reporting processes.Each person involved in an incident must complete their own report so the right follow-up and support can take place

WCB coverage

Students and employees are covered by the Workers’ Compensation Board (WCB) while participating in university-related work or academic activities. Recreational activities, such as Sport and Wellness programs, are not covered.

Completing an incident report automatically starts any required WCB process.

Concerns and complaints

Not every issue is an incident. Sometimes you may notice a safety risk before something happens, or you may need to raise a concern about conduct or behaviour. In such cases, different processes ensure your report goes to the right place.

Unsafe Conditions and Practices (UCAP)

Report a concern about any unsafe condition or practice that could pose a risk to health, safety or the environment before it leads to an incident. This includes issues like unsafe equipment, tripping hazards or improper chemical storage.

Faculty and staff

Report concerns through the Employee Health and Safety Concern form.

Students

Report concerns to the Security Services non-emergency line by calling 780-497-5554.

Complaints

A complaint involves conduct, harassment, discrimination or a violation of university policy. The Office of Investigations manages all complaints in coordination with Health, Safety and Environment, Human Resources, Security Services and Student Conduct.

All complaints are reviewed under MacEwan’s Investigations Framework, which ensures a fair, unbiased and transparent process.

Need to report anonymously?

If you wish to share a concern anonymously, use the Confidence Line to submit a confidential report. Incident, concern and complaint forms require contact information so the University can follow up properly.

Frequently asked questions

Once your report is submitted, it’s reviewed by the appropriate area, such as Health, Safety and Environment (HSE), Security Services or Human Resources, depending on the type of incident. You may be contacted for additional information. If follow-up or corrective actions are needed, the appropriate team coordinates those next steps.

No. Everyone involved in an incident is responsible for submitting their own report. Once your report is submitted, the system automatically notifies the appropriate supervisor, manager or faculty contact.

All injuries that occur on campus—including those related to Sport and Wellness activities—must still be reported through the University’s incident reporting process.

However, recreational activities are not covered under Workers’ Compensation Board (WCB). In addition to submitting an incident report, you should also notify Sport and Wellness staff so they can assist you with any immediate support or follow-up.

Yes. If you have new information or need to make corrections, please contact the area that received your report, such as Health, Safety and Environment or Security Services. They can update the report on your behalf.
